Understanding NYC Department of Health Inspection Grades

Inspectors conduct unannounced annual sanitary inspections under NYC Health Code Article 81. Points are assessed for each infraction, where a lower score indicates fewer violations:

  • Grade A (0–13 points): Cleanest regulatory compliance tier.
  • Grade B (14–27 points): Moderate sanitary infractions; establishment is permitted to operate while preparing for re-inspection.
  • Grade C (28+ points): Substantial sanitary deficiencies requiring corrective action and priority re-inspection.
  • Grade Pending: Assigned during initial or pre-permit cycles while an establishment exercises its right to a scheduled re-inspection or OATH hearing.
